Lead BH Navigator/MSW (LICSW)
The Lead Behavioral Health Navigator is responsible the operational management of the social, emotional and financial wellbeing of the program and patients identified as needing the services of the Behavioral Health Navigator roles. The Lead Behavioral Health Navigator is responsible for developing the patient discharge plan and executing this plan including coordination of the services required by the patient upon discharge. They are responsible for exercising appropriate assessment of the patient and providing any psycho-social intervention, as needed. The Lead Behavioral Health Navigator will accept referrals from the outpatient areas and refer to community agencies as necessary. The Lead Behavioral Health Navigator will provide education/consultation to the Nurse Managers and Nursing Staff to ensure achievement of quality patient outcomes, as well as other members of their team. The Lead Behavioral Health Navigator serves as primary liaison for ED Collaborative Care program aimed at coordinating community wrap-around services for frequent users of emergency services. Through screening, the social worker will provide early detection, brief intervention and referral for patients with alcohol and drug related health problems. They will also provide direct services to psychiatric patients of all ages in a hospital emergency department and inpatient setting, including evaluations and level of care assessments along with other duties as assigned.
Education: Master or Doctorate of Social Work, Psychology, Counselling or a related field
Experience: One to two years experience in clinical setting, experience in developing and facilitating patient discharges from an acute care setting, excellent assessment, diagnostic and crisis intervention skills.
Certification/Licensure: Current licensure in Massachusetts as a Licensed Independent Clinical Social Worker, Licensed Mental Health Counselor or Independently Licensed Behavioral Health Clinician
