Steward Medical Group at Hawthorn is looking for an enthusiastic, dedicated Dietician with a positive attitude, along with the ability to work both independently and on a team in a fast-paced environment.

The successful candidate will:

Assess patient nutritional status, diet history, food habits and determines risk level/needs. Confers with physician as appropriate on effect of nutritional status.

Provide patient/family counseling on appropriate nutrition and develops/implements plan of care. Follows up via phone and office visits.

Counsel patients on special diets and diet techniques to ensure proper preparation nutritional intake.

Provide dietary assessment/plan summary for medical record.

Plan and participate in patient education programs for weight reduction, diets for special needs including diabetes, high blood pressure, etc. Conduct health education programs for staff. Update educational materials

Team member at Diabetes Management Center teaching in individual and group settings. Involved in CQI and DMC

Other duties as assigned


BS in nutrition, food service, community health or health field; MS preferred; successful completion of American Dietetic Association RD program. ADA Registered Dietitian CDE or willing to sit for exam when eligible preferred.

Minimum of three years experience as dietitian; two years experience in outpatient setting preferred as dietitian/patient educator.

Knowledge of nutritional/dietetic principles and applications. Knowledge of concepts and methods of patient/staff education including teaching and educational aids. Knowledge of how to design special diets specific to a health condition
