Job Summary:

The position reports directly to the National Executive Director of Operations and is the lead member of the Chronic Care Management team (CCM) who works directly with SMG practices/providers to increase patient engagement around eligible patients for the program. This position acts as a liaison between the providers/practices with Steward Health Care Network (SHCN) who provides the CCM monthly services as directed by the Primary Care Providers. The CCM Operations Manager will ensure that the requirements to meet the success of CCM are met in each SMG primary care practice.


The CCM Operations Manager Functions:

  • Ensures all activities around the success of the CCM program are met.
  • Works directly with Practice Managers/Directors and Providers to ensure outreach is being done daily to all eligible scheduled patients to increase patient referrals to the CCM program.
  • Works with all practices assigned to ensure the weekly reports of scheduled appointments for eligible patients are worked each week by the practice staff.
  • Ensures all monthly charges are being entered into Athena by practice staff and will communicate with revenue cycle and coding as needed for success.
  • Works directly with SHCN staff to resolve any concerns or issues as they surface.
  • Updates patient and practice CCM material if any changes are made to the program.
  • Responds to all and questions coming from practice staff and providers to ensure all are educated to the correct workflows.
  • Attends weekly meetings to review the status of the program.
  • Works with new practices to coordinate training with the training department, coding and operations to ensure each will be present on the training to roll out program to new practices.
  • Works with practice managers to ensure all new staff and new providers in practice are properly trained on the program.
  • Other duties as assigned for the CCM program success.


  • 1 to 3 years of experience in a medical practice.
  • Medical terminology or basic medical knowledge.
  • Strong customer service background.
  • Experience with electronic medical record strongly preferred. (Athena a plus)
  • Experience with Microsoft Office, Word, Excel and Outlook.
  • Previous medical office manager experience preferred.
